Family Service Coordinator- Aftercare

Now seeking applicants for our expanding Aftercare program.
The position is generally Tuesday through Saturday with an evening shift on Saturday.
Responsibilities include:
· Engage with youth and families before discharge from Residential.
· Cooperate with treatment team to ensure successful transition to Aftercare.
· Provide exceptional case management services for youth in Bashor’s Aftercare Program.
· Lead Child and Family Team Meetings with multidisciplinary team.
· Provide in-home case management when warranted.
The ideal candidate will have:
· A bachelor’s degree in a human services field.
· An ability to complete tasks in a timely and thorough manner.
· Excellent verbal and written communication skills.
· A desire to make a difference in the lives of families.
Bilingual Spanish and English speakers who can help with translation needs are encouraged to apply!
Starting wage is $19.47/hour to $22.91/hour based on schedule and experience.
